their health insurance and insurance class. You can make an appointment, call or write an email directly online. Patients who have a family doctor model (HMO model) are asked for a referral from the family doctor.<\/p>\n<p>An examination or consultation appointment takes about 30 minutes. The referral should include the following information: last name, first name, date of birth, contact details of the patient and diagnosis.
